I was afraid of something like this......
quick search found this info on another forum and it sounds very logical/correct to me.
It's possible the fuel in your rear tank is never getting pumped into the front and you die showing 1/4 tank!

Here's the text:

There are two common problems with the dual tank systems. When the sensors wear, the gauge will no longer read full even though the sensors minimum (full) resistance often comes very close (3 ohms) to spec (40 ohms). The front and rear sensors are the same, just the float arms, which can be exchanged, are different. Also, when a pump fails there's confusion. The front pump supplies fuel to the engine so if it fails, or has no fuel, the engine won't run. The rear pump is only a transfer pump. When it fails the minimum fuel reading will likely be 1/4 tank. When fuel is added, and the rear tank is full, all the new fuel ends up in the front tank. Test the rear pump by activating it's relay which is attached to the side of the under hood relay box. So, the PCM controls both fuel pumps, transfering fuel from the rear tank to the front as needed based on the information supplied by the two fuel level senders
