The water pump running backwards would result in overheating, not cooler running.

Your symptoms are classic thermostat. Low temperature reading, low heat output in cold weather.

If you drill a hole in the flange, make sure it is small. 1/8" or 3/16" is more than enough. Any more and the truck won't warm up properly and you won't get heat in real cold.

: I'm sure it's just a thermostat. Make sure you buy a good one like Robertshaw or a dealer one. I would only look deeper if the stat is not even there (indicating previous owner band-aid of overheat condition). A small hole in the Tstat flange can help balance the Tsat and also keeps an air pocket from forming beneath the stat during re-filling.
