Brother-in-law makes tons of money selling these insurance polices as manager of large Ford dealership.
The depends questions: how handy are you with vehicles? How long are you going to keep the vehicle? How hard do you drive or work the vehicle?
If you are planning to sell, might increase the price and easy of sale as long as the warranty is transferable.
If really handy, you should consider your resources and if you think you can fix or knowledgably manage the repair there of. If not, then at the mercy of the service providers (get help from this board).
We bought a used 1998 Mazda B2500 pickup that had ~28K miles on it. Bought the extended warranty for just under $600 bucks (think it's a 3 year on top of the factory, which ended last year). Paid for itself already, as the AC compressor and accumulator went bad late last year...just outside of the factory warranty. Can't remember the name of the firm, but they are out of Florida with national chain of inspectors/associations. The AC shop said they were a bit tough initially asking questions and threatened to send out an inspector, but after they called me on how it went bad and when it confirmed with what the shop said, they gave authorization. They paid via credit card (my shop prefered that). The repair now has AC shops warranty on work, parts warranty and continues on vehicle warranty.
I'm very happy with it.
