Exhaust Manifold R+R

Need some help guys. I've ordered a new RH ex. manifold for my 91 R2500 7.4 burb and have questions.
1. Attempt to loosen bolts hot or cold?
If hot...engine hot or torch hot?
2. Reuse old bolts or new?
3. Tightening...torque specific or tighten the
beejesus out of it?
4. Tightening pattern... front to rear
or anything goes?
5. Gasket...sandwiched dry between components or
use Hight Temp. RTV?
6. Use hand tools or air tools?
7. Remove the donut and flange first or
get the manifold bolts loose 1st?

To date, all I've done is eyeball this repair and repeatedly soaked all fasteners in PB Blaster. Since Mr. Murphy is usually hanging around whenever I pick up a wrench, thought it best to get these questions answered, before Mr. Murphy gets involved.
