This was common with the old style GM cruise control. On my 89, when it was relatively new, it would seem to "go to sleep". Tap on the gas and it "woke up". It was the speed transducer (the part that the speedometer cable went through). A new one fixed the problem for as long as I owned the truck.

If I remember right, there is also a vacuum diaphragm that actually moves the throttle.

Same thing happened on my wife's 84 Olds Delta 88.

Lots better than an older style GM cruise control that put the pedal to the metal when it failed!

At 226 K you are lucky to be worrying about minor problems like this. Congratulations.

: Have '91 burb with 226K miles. when I engage cruise sometimes it holds and sometimes not.
: when it does grab it usually only holds for few seconds then slowly loses speed. If I tap the gas it will come back
