Some times it's not the steering box at all, but lose steering box mount bolts or even stress cracked frame rail where the box mounts. My 1973 K5 Blazer had a recall for just that and had a 1/4inch backing plate retrofitted to the frame rail on the other side of where the steering box mounted. I had already made my own when I got the recall notice because of my laziness using the tires to move stuff out of the way.

Not many will flush their power steering fluid, because street vehicles don't see as much stress there. 4x4's do, tons more from dirt/boulder/etc and larger dia/width tires/wheels.
